Order the Admission Form for this Patient
These records can help you trace your family tree and understand the lives of your ancestors. Order a copy of both the admission form and 'General Register' entry for £15.00. Watch our video to find out what you will receive.
For entries in this index after 1858 there is an entry for each patient in the 'Notices of Admissions by the Superintendent of
the Mental Institutions' (If the admission was prior to 1858, please use the 'General Register only' search below instead). For the research fee of £15.00 we will supply the entry from the 'General Register' and also locate the more detailed
admission form for you. These forms are held by the National Records of Scotland in Edinburgh. You can view an
example of the admission forms here.
As this involves a trip to the National Records of Scotland in Edinburgh it can take a few weeks to process your order. We usually try
to complete all orders within 4 weeks.

Order General Register Entry (only)
This search is for the 'General Register' entry only. To search for both the 'General Register' and more detailed asmission forms, see above. The research fee of £4.00 will include the provision of a digital image of the entry in the 'General Register' (NRS reference MC7) relating to the patient. Although the original record is brief it will contain the date and reason for why the patient left the institution if they left there before death. Once you pay for your order we will conduct the research for you as soon as possible and email the results to you, usually within 72 hours (please note this may take longer at busy times or evenings/weekends).
